Pre-Screening Questionnaire
Thank you for your interest in joining the Integrative Concierge Care Membership at NENH. We would be glad to learn more about you and your health goals. To begin, we kindly ask that you complete and submit our pre-screening questionnaire. Before doing so, we encourage you to review the Integrative Concierge Care page, which offers a fuller picture of what the program involves.
Once your questionnaire is received, Dr. Marinaro and her Medical Assistant, Erica Cunningham, will review it together to determine whether the program is a good fit for your needs. Their decision rests on two considerations: whether you have read and agreed to the guidelines of the Concierge Care Program, and whether the conditions you are seeking care for fall within Dr. Marinaro’s areas of specialty:
- Mast Cell Activation, Dysautonomia, and Hypermobility Syndromes
- Complex Chronic Infections, including tick-borne illnesses, Long-COVID, and ME/CFS
- Mold and Mycotoxin illness
- Autoimmune conditions
- Irritable Bowel Syndrome and Small Intestinal Bacterial Overgrowth (SIBO)
Should your questionnaire be approved, Erica will reach out within two to three weeks to schedule your first appointment, complete your concierge contract, and answer any questions you may have. To reserve your appointment, we ask for a deposit equal to your first month’s subscription. This deposit is fully refundable should you need to cancel at least five business days before your scheduled visit.
If we find that the Concierge Program is not the right match for your needs, Dr. Marinaro and Erica will be happy to recommend other naturopathic providers here in Maine. And if you would prefer a traditional practice model rather than concierge care, you are welcome to explore the searchable directory of naturopathic doctors at mand.org.
