If you are interested in joining the Integrative Concierge Care Membership offered by NENH, please fill out and submit a pre-screening questionnaire. Dr Marinaro will respond to you directly to initiate a dialogue and explain her approach to your particular complaints. If it is determined through this dialogue that you are a good match with the practice, you will be asked to fill out the below paperwork and submit electronically as part of your intake process. Please do not fill out the Intake Paperwork below until you have discussed your case with Dr Marinaro.
